Every project should have a quality assurance (QA) programme carried out by an independent assessor. Roles and responsibilities of internal participants or third parties must be clear and be consistently met. Clear milestones must be identified, constantly validated and tracked, and the project kept in line with the main objectives.
Projects can go off on a tangent. It's too easy for a project team to become wound up in ideas that are attractive but not core to success. It's also easy for a project to slip out of line with the business objectives.
